There is a shortage of diabetes educators and recognized diabetes education programs in this medically underserved state. There is a compelling need to scale up a community-based diabetes prevention program that can be delivered throughout the state in a way that is effective and sustainable. This project is innovative in three ways. First, little research has examined the effectiveness of a community-based DPM program among West Virginians. Second, community stakeholders were engaged in the process and provided feedback aimed at tailoring the program for participants. Third, we used our trained Health Coaches to provide education and receive feedback via follow-up weekly phone calls.
